Sales Exemption Form
Purchasing items for the PTO? Print this form and bring it with you when you are making a purchase on behalf of the PTO in order to avoid being charged state sales tax. If a seller chooses not to accept an exemption certificate, ask the seller to provide a properly completed 'Assignment of Right to Refund' so we can request a refund of the tax directly from the Comptroller. Another option is to ask the PTO President/Treasurer to purchase on your behalf from Amazon or Walmart as we have exempt accounts setup. **Sales tax will not be reimbursed if you fail to use the tax exempt form.
Reimbursements
Need to be reimbursed for approved purchases made on behalf of the PTO? In order to monitor and report PTO expenses timely, submit your requests within two weeks after incurring your expense.
